First Visit

New Patient Paperwork

Patients are able to visit our website and complete new patient forms electronically from a desk top or cell phone. If patients are coming in as a results of auto accident, there is one set of forms that Patients must print, complete and bring in at the new patient appointment.

If any patient has been in an automobile accident, patient has 3 sets of paperwork.

Depending on patients health history, most patients should allow 45-90 minutes for the initial appointment. Patient who have not completed the paperwork prior to their intial appointment should arrive at least 20 minutes prior to the scheduled appointment time to complete the paperwork. The paperwork includes information that provides Dr. Hastings with general health history information, present concerns and conditions. Patients also sign acknowledgement of HIPAA and provide individual consent to treatment and filing with insurance, if applicable. Please bring a copy of your photo ID & insurance card.

Consultation

Once the paperwork has been completed, you will have a consultation with the doctor to discuss your health-related problems, concerns, as well as potential treatment options.

History and Examination

In order to determine what your actual problem is, the doctor will ask you various questions related to your condition. Next, a number of specialized tests will be performed to determine which structural areas and/or soft tissue areas are affected and to what extent.

X-ray Studies

Your specific condition may require us to complete x-ray examination to either rule out more serious conditions or assist us in developing the most effective treatment plan for you.

Same Day Treatment

Once finished with the examination and all appropriate studies, patients will generally be provided with their first treatment during the same initial visit. Dr. Hastings does reserve the right to postpone any treatment on the first visit based on a patient's health history and present conditon. Additional time may be needed to review findings before a treatment is performed. Please allocate the time noted above for the initial visit. If a treatment were to be received, it may include spinal adjustments, physical therapies, and/or soft tissue therapies.

Home Instructions

Prior to leaving, patients will be given instructions on certain activities or procedures to be conducted at home. This may include ice or heat application instructions, avoidance of certain activities or positions, as well as home exercises and/or stretches.

Schedule Your Next Appointment

You will be given a convenient appointment time for the second follow-up visit. Generally speaking, patients are seen within 1-2 days and are provided with a complete review of findings. Treatment options for your condition will also be discussed during this time.
